New Patient Registration
We invite prospective new patients to come down to the Health Centre to fill in a Registration Form (for patients over 16). One person can fill in a Patient Registration Form for each family member. You will also need to bring with you two forms of ID for all patients over the age of 16 (ID Requirements). If you are registering a new baby, please complete a Newborn Registration Form.
If you have a Long Term Condition such as Diabetes, Asthma, COPD etc. our registrations clerk will contact you to inform you of the services we offer in terms of your care.
For patients taking medication, once registered we ask that you make an appointment see a GP when your first prescription from us is due.
In order to register you must live within the practice area. As our practice population continues to grow due to the increasing number of building developments in the area, we have made the difficult decision to change our practice boundary from 1st Spetember 2020.
For patients who are currently registered with the practice whose current address is in one of the areas no longer in our boundary, we would like to reassure you that you will not be asked to move GP and you will still be able to access services from Castle Medical Group.
Our boundary covers the following areas:
- Boundary (as far as the Water Tower)
- Moira (BUT NOT including Shortheath or the Donisthorpe side of Measham Road and Moira Road)
The following areas are not eligible to join our practice:
- Some areas of Moira - We do not accept new patients from the Donisthorpe side of Measham Road and Moira Road
- Norris Hill - including Boothorpe and the Conkers side of Blackfordby Lane
If an existing patient moves to a new home which is outside our practice boundary we will have to ask you to register with another practice. Due to the pressures of new housing leading to our list size increasing constantly, we are unable to make any exceptions to this rule.
