All new patients over the age of 16 will need to provide identity documents to register as a permanent patient.

We need:

1. 1 photo ID from group 1 below
2. 2 documents from group 2 below (1 of which must have a current address and be no more than 3 months old)

Group 1

• Valid Passport
• UK Photo Driving licence
• EU Photo Driving licence
• MH Forces Identity card
• Student Identity card
• Buss pass (with photo ID)

Group 2

• Marriage licence
• Financial statement, e.g. mortgage statement
• Birth certificate
• Vehicle registration document
• UK P45 or P60
• Mail order statement
• Bank/Building society statement
• Utility bill
• TV Licence
• Addressed payslip
• Credit card statement
