We do not offer SystmOnline online access for patients in this age group. We direct patients of this age to register with the NHS app.

Proxy access can be granted without completing a form, please email us at castlemedicalgroup.noreply@nhs.net to request this, including yours details and the details of the person you would like proxy access too.

To authorise someone else to discuss your medical records with us, please complete this form and ensure it is signed by you, the patient, and presented to our practice staff in person with ID, or else it will not be processed.
